Marcus Smart's mother Camellia Smart died in September of 2018. That same month, Smart participated in a video interview with NBC Sports Boston where he discussed his mother: "You know, I got a new tattoo because of her. I got her name, the date she passed away, and a saying we used to say all the time before we got off the phone: 'God love you, I love you.' So that was for her. I'm just going to go out there and continue to do what I've been doing. I love her, she's gone but never forgotten."

Just before the start of the 2015-16 season, Smart added two lions to his left forearm. The tattoo is by Dee Corsey of Dallas, Texas.

When Smart was ten years old, his older brother Todd passed away after a long battle with cancer. Todd wore jersey #3 in high school, so Marcus’s first tattoo is the number three on the back of both of his arms.
